| Video Discription | Temporary Uterine Artery Ligations for Minimizing Bleeding in Laparoscopic Resection of Cesarean Section Scar Pregnancy Since 1996, the cesarean delivery (CD) rate in the United States has increased by approximately 40%, and in 2007, the rate was 31.8%. A previous CD increases the risk for a pathologically adherent placenta (accreta, increta, percreta and cesarean scar pregnancy). A removal of cesarean scar pregnancy surgery is very challenging for conventional laparoscopy and is well known for hemorrhage. Hence, this video demonstrates how to perform a laparoscopic removal of a CSP using harmonic Scalpel with temporary uterine artery ligations. This patient is a 32 YO G3P2002 who was planning termination reportedly 6w5d pregnancy within the uterine scar from two previous cesarean sections per ultrasound. Beta hCG was 51, 558 mIU/ml with Cardiac activity of 134 bpm. MRI and Pelvic US both showed: a gestational sac is visualized in the anterior/lower uterine segment and eccentrically placed within the myometrium (prior CS scar); There is marked thinning of the overlying myometrium with hyperintense (decidua basalis) abutting and indistinguishable from the superior bladder wall (2.6mm); Gestational sac measures 1.8 x 1.4 x 2.5 cm. In this case, the gestational sac was evacuated from CSP after temporary uterine artery ligations with minimal blood lost. The patient was discharged same day without complication. In conclusion, surgical removal of gestational sac with temporary uterine artery ligations can result in shorter hospital stay and rapid decrease in BHcg concentrations and provide decreased blood loss, ability of diagnosis confirmation, repairing defect in uterus and reducing risk of recurrent. |
