| Video Discription | Case Report 31 one year old female underwent Laparoscopic cholecystectomy for Acalculous Cholecystitis in March 2020. On post-operative follow-up day 15 patient developed altered LFTs and patient had jaundice on 1 month follow up. There was no fever. Preoperative Evaluation and preparation – On evaluation, Patient had Obstructive Jaundice and Type 4 Bismuth Biliary stricture (Separation of Rt and Left hepatic ducts) on MRCP and CT scan abdomen. Pre-operative PTBD was done in Right duct and contrast couldn’t enter left duct or cbd. Surgery – Right subcostal incision was given. After opening peritoneum, careful adhesiolysis was done and Hilum was exposed. Hilar plate was lowered and left duct was exposed. Clip was removed from hilum. Hilum was opened towards left duct for 3 cms. Wide Roux-en-Y hepaticojejunostomy was made at hilum. Post-operative course – Patient’s RT was removed on POD – 1. Patient was started on liquids and progressed to solid diet over next 2 days. Drain was non bilious and it was removed on POD – 4. Patient was discharged without any complications. PTBD catheter was removed on 2 months follow up. Patient is on regular 3 monthly follow up with USG, LFT. Key words – Bile duct stricture, Hepatico-jejunostomy |
