| Video Discription | This case shows a 55-year old lady with a progressive third nerve palsy on the right side six months earlier. Diagnostic workup showed an aneurysm of the posterior communicating artery on the right side. She was treated endovascularly with coiling, which resulted in a perfect coil-reconstruction. At 6 months follow-up, however, the aneurysm showed recanalisation at the neck. In multidisciplinary neurovascular consultation and shared decision making with the patient we concluded to perform a surgical clip reconstruction. The surgery was performed through a right-sided pretemporal approach with extradural removal of the anterior clinoid process. Through this, an excellent proximal and distal control of the aneurysm was available. The aneurysm was clipped adequately and the patient recovered well from surgery. Surgery by: drs.
