Middle ear exploration reveals the presence of a severe tympanosclerotic involvement of the malleus and a dislocated incus transposition. Dissection of tympanosclerosis which is entirely removed allowing for malleus relocation. There is a mobile stapes footplate which is partially eroded but with intact perilymph membrane which is preserved. Vein graft interposition.
