| Video Discription | This 58-year old man had a previous history of a giant right-sided middle cerebral artery bifurcation aneurysm that was treated endovascularly 4 and 2 years earlier. The aneurysm was detected because of epileptic seizures. Unfortunately, the aneurysm showed refilling and growth because of coil migration into the thrombus and epilepsy recurred. After multidisciplinary consultation we decided upon surgical treatment of the aneurysm. For this, a double barrel STA-MCA EC-IC bypass was performed after which the aneurysm could be isolated from the circulation and cleaned from coils and thrombus. The aneurysm could then successfully be clipped and the normal cerebral circulation was restored. The patient suffered a quandrantanopia that recovered partially.
